The Expeditious Community seems to rally around the Mitsubishi cabover over the Izusu cabover, or the Chev cabover, or any of the other middleweights. Why is this?

There's also Hino, but the new ones are not cab forward design. 2004 and before are.

Like FusoFG said, the Fuso FG is the only cab-forward 4x4 truck sold in North America. (I don't think of the U500 as a cab-forward model, it's in its own category.)

You can purchase heavy trucks converted to 4x4 from Pioneer 4 Wheel Drive (Nissan UD 3300 and 2200 models)

and from RUF-Inc (Mitsubishi FM260)

Isuzu (note the spelling) has several great 4x4 models in its cab forward line, including smaller 4 cylinder trucks with single rear wheels, and a larger truck with 6 cylinder engine often used by fire brigades in Japan and Australia. But they don't import them to USA.

If you're not concerned about the advantages of cab forward, then there are several medium duty pickups and cab/chassis models from domestic manufacturers to consider.
